Treatment plans are critical to ensure that the HVRC clinical team and clients are thinking through all the important aspects necessary for each client’s recovery. Meanwhile mapping out the course of each client’s treatment. HVRC treatment plans contain both short and long-term treatment goals, and a plan for how those goals will be achieved. Treatment and Recovery plans are individualized, meaning that treatment plans should be tailored to the client, and contain different measurable goals and action steps for each client. HVRC individualized client-centered care assumes that different clients require different services and treatment approaches in order to meet their unique needs.

HVRC Treatment Planning Process

The treatment planning process begins with the assessment and intake process, where the information is collected to form the basis of each client’s initial treatment plan. A treatment plan of action based on client’s presenting problems, physical behavior and emotional status, and mental wellness is developed and agreed upon within 72 hours of intake.

All HVRC treatment plans are individualized or based on the client’s particular problems and needs. This road map to recovery identifies each client’s strengths and limitations and also involves family where appropriate.

See a detailed list of HVRC’s treatment planning steps below.

HVRC Treatment Planning Steps

In order to create all HVRC treatment plans, the following steps are taken.

To create the treatment plan, the client and the primary counselor or therapist lists the significant problems or issues that the client is experiencing. Then rank problems and issues in terms of priority.
Once the problems are identified, the client and their counselor or therapist come up with long term and also short term goals.

Treatment goals should be expressed clearly and in concrete, measurable, behavioral terms, and include expected date of achievement.

Specific counseling strategies and services are then identified, as well as who will perform the tasks, where they will take place, and how often. Outside referrals may be considered.

Client progress notes should regularly reflect attention being paid to the areas outlined in the treatment plan – documentation of progress and struggles in these specific areas are noted regularly.

Resolution of problem areas specified in the treatment plan are documented in either a progress note or the treatment plan itself.

HVRC treatment plans are reviewed by our board certified medical director, the client, and the primary counselor or therapist, on a weekly basis (every 7 days). Constant and consistent review is important to allow for flexibility and changes in addressing client problems, needs, and goals.

HVRC develops written and individual treatment plans for all Clients admitted. HVRC treatment plans serve as a roadmap to guide individuals from the assessment process through to successful program discharge. Using a goal and action-oriented process with measurable criteria created to produce the desired outcome and results, plans are designed to keep both the clinical team and our clients focused on achieving their targeted objectives and goals.
