Treatment plans are critical to ensure that the HVRC clinical team and clients are thinking through all the important aspects necessary for each client’s recovery. Meanwhile mapping out the course of each client’s treatment. HVRC treatment plans contain both short and long-term treatment goals, and a plan for how those goals will be achieved. Treatment and Recovery plans are individualized, meaning that treatment plans should be tailored to the client, and contain different measurable goals and action steps for each client. HVRC individualized client-centered care assumes that different clients require different services and treatment approaches in order to meet their unique needs.
HVRC Treatment Planning Process
The treatment planning process begins with the assessment and intake process, where the information is collected to form the basis of each client’s initial treatment plan. A treatment plan of action based on client’s presenting problems, physical behavior and emotional status, and mental wellness is developed and agreed upon within 72 hours of intake.
All HVRC treatment plans are individualized or based on the client’s particular problems and needs. This road map to recovery identifies each client’s strengths and limitations and also involves family where appropriate.

HVRC Treatment Planning Steps
In order to create all HVRC treatment plans, the following steps are taken.
HVRC develops written and individual treatment plans for all Clients admitted. HVRC treatment plans serve as a roadmap to guide individuals from the assessment process through to successful program discharge. Using a goal and action-oriented process with measurable criteria created to produce the desired outcome and results, plans are designed to keep both the clinical team and our clients focused on achieving their targeted objectives and goals.
